Job Description:
RN Field Case Manager assesses, observes, evaluates, and administers skilled nursing care and teaches under a physician's plan of treatment to meet needs of the agency's patients in their homes.
RN Field Case Manager is responsible for the total management/complete coordination of services provided to assigned patients.
Communicates regularly with patients and their families about Plan of Care, PT/OT/ST, Aide, MSW and other treatment protocols.
Answers patients questions about their care, treatment plans, illness progression and all other issues so they feel safe in our care.
RN Home Health Field Nurse qualification's and skills:
Graduate of an accredited School of Nursing.
Current RN license by the State of Indiana.
Maintains current CPR certification and other skills required by the home health agency policy.
Must have 2
years experience in acute care setting as an RN prior to Home Care or experience as required by State regulations.
2-3
years experience in Home Health preferred but not required.
